<p><strong>How it Works</strong></p>
<p>For some time, scientists have known that probiotics promote good digestive health. Yogurt, a food rich with probiotic power, is often recommended for patients with stomach aches and digestion issues. The same philosophy backs the effectiveness of Evora.</p>
<p>Good oral bacteria “crowd out” bad bacteria to promote a positive balance in the mouth. In addition, by reducing bad bacteria, Evora mints reduce potential for foul breath caused by bad bacteria. Good bacteria actually bond to teeth and gums, and even below the gum line for lasting results. This is unlike traditional mouthwashes that kill all bacteria in the mouth, not just the bad bacteria.</p>

The American Academy of Cosmetic Dentistry (AACD) has estimated that the demand for tooth whitening has increased by 300% over the past decade. Tooth whitening is the number one dental procedure requested by people under the age of 20 and between the ages of 30 and 50.  Initially, tooth whitening was performed in the dentist’s office and this might require several visits.  The process has become simpler and is now offered as an over the counter product allowing people to perform their own tooth whitening.</p>
<p>Two companies, Procter &amp; Gamble and Colgate Palmolive, dominate the market for over the counter tooth whitening products.  As of 2003, these companies had about 84% of the market.  Americans spent about $297 million on home use tooth whitening products during the year ending March 23, 2003.  This was an increase of 90% over the prior year’s sales. Each company has introduced additional brands of tooth whitening products for use at home.  The names of some of the major brands are provided in Table 1.<span id="more-3553"></span></p>
<p>Other companies have been entering the home use market.  These include Bident, Mentadent, and Rembrandt.  The overall market for tooth whitening products grew by 596% between 2000 and 2005; in 2005 there was a decline of 23% because of the introduction of other products.<a href="http://www.dentalblogs.com/assets/table.jpg"><img class="alignright size-medium wp-image-3554" title="table" src="http://www.dentalblogs.com/assets/table-300x130.jpg" alt="" width="459" height="198" /></a></p>
<p>Overall cosmetic dentistry has surged with growth rates of 12.8% in 2006 and 10.9% in 2007.  The demand for bleaching and whitening of teeth has been one of the top requested services.  However, the cost of using a dentist has become too high for some people and the result is the growth in over the counter products.  With the increases in the older population, the need to look young has become even more important. And having a bright white smile is a major part of “looking young.”<br />

<p>By altering skin tone and tooth brightness on photographs of smiling women, researchers created a way to test how people perceive beauty in regards to tooth enamel color. Seventy dentists and 70 laypeople answered questions about the photos. The findings show that skin color affects the shade that is attractive on teeth.</p>
<p>According to the report:</p>
<blockquote><p>“Dentists and laypeople did not perceive the brightest tooth shade to be the most attractive, and they did not perceive all skin colors to be equally attractive with bright white teeth. Respondents perceived dark skin with bright white teeth and fair skin with dark teeth as relatively unattractive.”</p></blockquote>

<p>What&#8217;s the point, then? The study&#8217;s findings provide reason for manufacturers to tweak home whitening systems and improve future generations of the products.</p>
<p><span id="more-2727"></span></p>
<p>While other studies have tried and failed to measure enamel loss, this recent study by researchers at The Ohio State University measured enamel reduction to the billionth of a meter using a nanometer scale, rather than in microns, which measure to a millionth of a meter. Enamel loss ranged from 1.2 to 2 nanometers after home teeth whitening was applied. It seems that white strips tend to reduce enamel less than tray methods.</p>

<p>Hey you guys here is a new twist. I was reading in HEALTH magazine that there is a dentist in Boston who makes &ldquo;tooth whitening house calls&rdquo;. Right in your own home he will take impressions of your teeth, make whitening trays and leave you with all you need to brighten your smile without leaving your own kitchen. He even offers to come to your office on your lunch hour to get you on the way to a whiter smile. Pretty different approach, I wonder how many he converts to actual patients in his office or if he even has an office??? He only charges $300.00 for this custom service. Sure sounds like a terrific deal for those too busy to get to our offices.</p>

<p>It is time of the year when in some practices things slow down. I have never really figured out why; maybe because of tax time or maybe it is an after the Holiday lull. There have always been things you can do, chart audits, special discount offers, email reminders of unfinished treatment plans, etc. Well here is a new one. Every existing patient who is being seen in hygiene for re-care take a shade and talk about whitening. Explain to your existing patients on their re-care visit that the &ldquo;standard of care&rdquo; in dentistry is changing. That you are now taking a full series of digital photos on all new patients when they enter your office. Get them into an empty chair and get photos on them. These patients of record deserve to see their mouths the same way new patients are seeing their mouths. It opens their eyes to the need to get some the dentistry they having putting off for years done. Give them the opportunity to have their comprehensive exam brought up to &ldquo;the new standard of care&rdquo; in your office.</p>

			<content:encoded><![CDATA[<p>We have all been on other forums where anyone can post anything about anyone. This often leads to angry and self-ritious grandstanding. In other words, confrontation for the sake of protecting ones turf, as opposed to simply sharing ideas for the simple sake of helping others. All the authors of this forum have the obligation and responsibility to maintain an environment where anyone feels comfortable asking a question or sharing a view without being ridiculed or interrogated. There are too many forums where this mentailty exist. The end result is that only a few meaningful post are presented which are then attacked and no one else post anything. That will not happen here. If you have any question or want to share an idea in a thoughtful way, please do so. That is what this forum is about. We have the best and brightest here and they truly want to help others achieve the same success that they have. They willingly share their ideas and experiances without any form of compensation other than the warm feeling of knowing they made a difference. 			<content:encoded><![CDATA[<p>&nbsp;&nbsp;&nbsp; The interest in bleaching teeth has continued to grow, particularly in North America. Although the process of &quot;whitening&quot; teeth has been around over a decade and a half, little is known about the mechanism. The same can be said about the postoperative sensitivity (POS) that commonly accompanies the procedure.</p>
<p>&nbsp;&nbsp;&nbsp;&nbsp; The mechanism of bleaching is actually attributed in part to the dissolution of enamel rod prisms. Based upon a Brazilian study it has been shown that the carbamide peroxide attacks and totally dissolves isloted groups of enamel rod prisms; thereby exposing the underlying dentin. The whitening that one observes during the bleaching process is actually due to chromatic changes in the dentin more so than in the enamel.</p>
<p>&nbsp;&nbsp;&nbsp; Now since an open avenue exists between the facial surface of the tooth and the surface of the dentin, the potential for POS can occur. As both patient and clinician recognize, the teeth undergoing bleaching can experience considerable pain upon the introduction of cold liquids.</p>
<p>&nbsp;&nbsp;&nbsp; Specifically, the cold water us transfered through the open channels (previously occupied by the enamel rod prisms) contacting the exposedodontoblastic processes. These odontoblastic processes are then subjected to a reduction in pressure which according to Brannstromm (1972) thereby creating pain. This pain can be quite acute. Incidentally, the dissolution of enamel rods has been demonstrated&nbsp; using scanning electron microscopy (SEM). Addional numerous other studies have demonstrate that bleaching reduces the microhardness of the enamel surface. Over a period of time the enamel fortunately remineralizes and the hardnes goes back up. Unfortunately some studies&nbsp; have demonstrated that the microhardness and remineralization may not necessarily return to the orginal pre-bleached state.</p>
<p>&nbsp;&nbsp;&nbsp; Now that the mechanism of bleaching has been established, better guidelines can be generated for the purpose of enhancing the effectiveness of the bleaching action. This knowledge also is quite helpful in prescribing a regimen for reducing&nbsp; or eliminating POS. For example, the home bleaching procedure can be coupled with the application of desensitizing agents such as GLuma ir Benzylalkonium chloride. <strong>Finally, the rate of enamel rod dissolution&nbsp; and achievement of the bleaching effect is related to concentration of the peroxide as well as the time of application.</strong></p>
